Delivered by a qualified My Care Solution caregiver, the Hospital to Home Essentials Package is a 2-hour service that includes all the basic care and support you need to settle in at home.

Your caregiver will collect you from your hospital room, pick up your prescriptions, do a light grocery shop and transport you home. There, your caregiver will change your bed linen and prepare a light and nutritious meal.

With no lock-in contracts and no hidden fees, our Hospital to Home Essentials Package is the ideal solution for a safe and supported return home.

The Hospital to Home Essentials Package can be paid for on a private fee-for-service basis, through your Home Care Package or through your NDIS plan if you are self-managed or plan-managed.

$95 Hospital to Home Essentials Package

Delivered by a qualified Caregiver, your Hospital to Home Essentials Package includes the following services:

  • Discharge assistance
  • Prescription collection
  • Basic grocery shopping
  • Transport home
  • Change of bed linen
  • Preparation of light meal

The package cost of $95.00 includes a total of 2 hours of service and 20 kilometres of transport. Additional hours and kilometres are available to purchase at the standard rate. Clients must be able to transfer in and out of a standard vehicle with assistance from a caregiver.

One of our Client Care Coordinators will contact you to discuss your care needs and requirements relating to the $95 Hospital to Home Essentials Package. If time allows, our Client Care Coordinator will provide a bedside consultation.

Once our Client Care Coordinator gain a better understanding of your situation, they will discuss your discharge with the hospital and select a suitable Caregiver to deliver your package.

On the day and time of your request, our Caregiver will collect you from your hospital room.

Next, your prescriptions will be collected and a basic grocery shop will be completed to ensure you have the necessities at home.

Once you get home, our Caregiver will change your bed linen and prepare a light meal.
