With dedicated support from your own Client Care Coordinator and a small team of qualified caregivers, My Care Solution will work with your hospital and other professionals involved in your care to make sure you are well taken care of.

Your Client Care Coordinator will develop and manage a tailored plan of care in partnership with you and your hospital discharge planning staff to start you off on your road to recovery.

Your nurse-led team of caregivers can assist you with medication support, meal preparation, housekeeping, personal care, and transport services to your follow-up appointments. Home nursing services are also available for your more complex care needs.

With no lock-in contracts or commitments, My Care Solution offers flexible hospital to home care options that allow you to increase or decrease your support as you see fit.

Our hospital to home care services can be paid for on a private fee-for-service basis, through your Home Care Package or through your NDIS plan if you are self-managed or plan-managed.
